What All-on-4 Actually Means

Instead of placing one implant for each missing tooth (which would require 10-14 implants per arch), All-on-4 uses four strategically angled implants to support a complete bridge of 12-14 teeth. The back implants tilt at 45 degrees to maximize contact with existing bone: usually avoiding the need for bone grafting. You get temporary teeth attached to these implants. After 3-6 months of healing, you return for your permanent bridge. It’s fixed in place. It does not removable like dentures, so it feels and functions like real teeth.

All-on-4 Dental Implants

When do we recommend All-on-4?

All-on-4 usually is the optimal treatment when:

You’ve lost most or all teeth in the upper or lower arch

Wear dentures that slip, click, or cause sores

Have advanced gum disease causing multiple tooth loss

Face full tooth extraction due to decay

Want to avoid the hassle of removable dentures

All-on-4 works even if you’ve been told you don’t have enough bone for traditional implants. The angled placement and strategic positioning often eliminate the need for bone grafts.

How the All-on-4 Process Works: Step-by-Step

Most people need two trips spread over 3-6 months. Budget about a week for each visit.

Step 1

Consultation & 3D Planning

Your dentist takes CBCT scans and creates a digital treatment plan showing exactly where each implant will go. You’ll see a simulation of your new smile and get precise pricing before anything starts.

Step 2

Surgery & Temporary Teeth (4-6 hours)

Under sedation or local anesthesia, the surgeon extracts any remaining teeth if needed, places four implants per arch, and attaches a temporary bridge. You leave with a full set of teeth.

Step 3

Healing at Home (3-6 months)

The implants fuse with your jawbone through osseointegration. You’ll eat soft foods for the first 2-3 weeks, then gradually add more texture. Virtual check-ins with your dentist track progress.

Step 4

Return Visit: Permanent Bridge

After healing is complete, you come back to Vietnam for 3-5 days. Your dentist removes the temporary bridge, takes final impressions, and installs your permanent zirconia or porcelain bridge. This is what you’ll use for the next 15-20 years.

Your All-on-4 Questions Answered

Am I too old for this?

Age isn’t a barrier. We’ve successfully treated patients in their 80s. What matters is bone density, general health, and healing capacity. Your dentist assesses this during consultation.

Does it hurt more than single implants?

You’re sedated during surgery. Afterward, expect swelling and soreness for 3-5 days. It will be manageable with pain medication. Most patients say it’s less painful than they feared.

What if I don’t have enough bone?

All-on-4’s angled implants work with less bone than traditional methods. Severe bone loss might require zygomatic implants (anchored in cheekbone) instead, which costs $13,000-15,000 per Zygomatic implant.

Can I get both arches done at once?

Yes. It’s a longer surgery (6-8 hours vs 4-6), and recovery is slightly tougher, but doesn’t extend the overall timeline. Many patients prefer doing it all in one trip.

What can I eat with temporary teeth?

Liquids and soft foods for 2 weeks. Think soups, smoothies, scrambled eggs, mashed potatoes. After that, gradually add soft pasta, cooked vegetables, tender fish. Avoid hard, crunchy, or sticky foods for 3 months.

When can I go back to work?

Plan on taking 3-7 days off. Desk jobs? Most people return after a week. Physical labor? Give it 10-14 days.

How long do All-on-4 implants last?

The titanium implants can last 20-30+ years, sometimes a lifetime. Your bridge will need replacement after 10-15 years from normal wear.

How do I clean them?

Brush like regular teeth twice daily. Use a water flosser to clean under the bridge. Visit your dentist twice yearly for professional cleaning. No removable parts.

What’s the success rate?

95-98% when done by experienced surgeons. Failure usually happens in the first 6 months if osseointegration doesn’t occur. Smoking dramatically increases failure risk.
